Brett Eldredge re-recorded his hit song, “Raymond” with a very touching and special tribute to his grandmother for the Alzheimer Association’s “Music Moments” album.

Brett shares stories of some of his first performances in his church, and how when he first got his start in country music, one of his biggest fans, his grandmother, was losing her memories day by day.

“I remember that feeling of, ‘How do I even make sense of this’? Eldredge says in a new video by the Alzheimer’s Association exclusively reveals to PEOPLE.

As she was fighting to keep her memories alive, he was fighting to keep his emotions in line as he continued throughout his career. “I wrote and tried to work through it and cope with it in that way.” The ‘Love Someone’ singer mentions.

He wrote and called the song “Raymond”, and dedicated the song to his grandmothers and to the 5.8 million Americans that are living with Alzheimer’s and their caregivers.
